JAC Assist

JAC Assist is a curb-to-curb service for eligible persons with disabilities who cannot use the regular bus system. Eligibility for the specialized service is defined by the Americans with Disabilities Act (ADA) of 1990. An approved application, verified by a licensed medical professional, is required to become certified for JAC Assist service. JAC Assist Application

The individual who wishes to be certified for the service will complete Part A and the Disclosure of Protected Health Information Authorization Form, while the medical professional will complete Part B.
